Diabetic heart disease

Abstract
Diabetes mellitus is responsible for a spectrum of cardiovascular disease. The best known complications arise from endothelial dysfunction, oxidation, inflammation and vascular remodelling, and contribute to atherogenesis.[1] However, the effects on the heart also relate to concurrent hypertensive heart disease, as well as direct effects of diabetes on the myocardium (Figure 1). Despite the description of diabetic cardiomyopathy by Rubler over 3 decades ago[2], the existence of this entity has been the source of ongoing controversy. For the purposes of this discussion, diabetic heart disease will be defined as myocardial disease in diabetic subjects that cannot be ascribed to hypertension, coronary artery disease or other known cardiac disease.
