Notable by its absence from the list of predictors of severe rheumatic valvar disease[1] was the mention of the role of gender, especially when one considers the fact that rheumatic mitral stenosis is reportedly twice as common in females as in males.[2-4] This was also a missed opportunity to test the hypothesis that there is a role for oestrogen in the expression of rheumatic mitral stenosis.
