A previously fit 24-year-old man presented to hospital with an episode of syncope while lifting a heavy load. Physical examination, ECG and echocardiography were consistent with hypertrophic cardiomyopathy (HCM). The potential cause of his syncopal episode included an outflow tract obstruction, arrhythmia, or simply, isometric exertion.
