Hypercholesterolaemia was defined as: total cholesterol >240 mg/dl; diabetes as fasting glycaemia >126 mg/dl on at least two occasions; hypertension as blood pressure >140/90 mm Hg; smoking as a smoker of > 1 cigarette per day at the time of admission; family history as documented acute coronary syndrome before 60 years of age in at least one first degree relative.
