AMI, acute myocardial infarction; BMI, body mass index; CABG, coronary artery bypass grafting; cerebrovascular disease, history of stroke or recurrent transient ischaemic attacks, or an internal carotid artery stenosis of ⩾ 70%;emergency, procedure required within 24 hours of referral; current smoker, smoking within one month of procedure; hypertension, systolic blood pressure ⩾160 mm Hg, diastolic blood pressure ⩾90 mm Hg, or current antihypertensive drugs; LMS, left main stem; PTCA, percutaneous transluminal coronary angioplasty;severe respiratory disease, forced expiratory volume in one second (FEV1) < 1.5 l, FEV1 ⩽75% of predicted, or regular use of bronchodilators or corticosteroids; urgent, patient unable to be discharged home between referral and operation on clinical grounds.
