CABG coronary artery bypass grafting;cerebrovascular disease, history of stroke or recurrent transient ischaemic attacks, or an internal carotid artery stenosis of ⩾ 70%; current smoker, smoking within one month of procedure;hypertension, systolic blood pressure ⩾160 mm Hg, diastolic blood pressure ⩾90 mm Hg, or current antihypertensive drugs; IHD, ischaemic heart disease; PTCA, percutaneous transluminal coronary angioplasty;severe respiratory disease, FEV1<1.5 l, FEV1 ⩽75% of predicted, or regular use of bronchodilators or corticosteroids.
