Whereas the left ventricular reserve (defined as the exercise induced increase in LVEF of at least 5%) did not differ in those with and those without fixed septal defects, the RV reserve (RVEF increase of at least 5% on exercise) was substantially impaired in patients with septal involvement [12]. Acute right ventricular impairment with associated hypotension is not infrequent in the immediate postoperative period in patients with longstanding pulmonary hypertension due to mitral valve disease who undergo mitral valve surgery.
