Hemodynamic alterations in idiopathic hypertrophic subaortic stenosis induced by sympathomimetic drugs

Abstract

The hemodynamic alterations induced by isoproterenol and methoxamine in 7 patients with idiopathic hypertrophic subaortic stenosis are described. The intravenous administration of 2 μg. of isoproterenol generally lowered the cardiac output and the systemic arterial pressure, while elevating the left ventricular systolic and end-diastolic pressures. Thus, this drug consistently decreased the effective orifice within the left ventricular outflow tract. In contrast, the administration of 2 mg. of methoxamine abolished the left ventriculoarterial pressure gradient in 3 patients. Variations in the severity of obstruction dependent upon alterations in intraventricular conduction were also demonstrated. The mechanisms responsible for the observed changes were discussed.
