Effects of Dobutamine on Aortic Valve Indexes in Asymptomatic Patients With Bileaflet Mechanical Prostheses in the Aortic Valve Position

The Sorin Bicarbon prosthesis (Sorin Biomedica, Saluggia, Italy) is a bileaflet valve with curved-profile leaflets, a rolling hinge mechanism, and a pyrolytic carbon—coated titanium alloy housing and sewing ring. Although the Sorin Bicarbon prosthesis has been implanted in greater than 80,000 patients, and reference values on the hemodynamic performance of valve prostheses are needed to avoid patient-prosthesis mismatch, few Doppler echocardiographic data are available on the prosthesis in the aortic position. The aim of this study is to provide a detailed echocardiographic evaluation of the hemodynamic performance and regression of left ventricular hypertrophy after aortic valve replacement with the Sorin Bicarbon prosthesis.

The study included 182 patients who received a 21-mm (n = 61) or 23-mm (n = 121) Sorin Bicarbon prosthesis for pure or prevalent aortic stenosis who underwent serial echocardiograms at 3, 6, and 12 months after aortic valve replacement.

Mean and peak gradients significantly decreased (P < .001) during follow-up to values of 12 ± 3 and 22 ± 6 mm Hg for the 21-mm prosthesis and values of 11 ± 4 and 19 ± 6 mm Hg for the 23-mm prosthesis at 1 year. Left ventricular mass index showed a 17% decrease to 120 ± 27 g/m² in recipients of the 21-mm prosthesis (P < .001) and a 21% decrease to 123 ± 29 g/m² in recipients of the 23-mm prosthesis (P < .001). A larger prosthesis size was the only predictor of a higher left ventricular mass index regression. Among recipients of the 21-mm prosthesis, body surface area of greater than 1.85 m² was associated with a lower regression of left ventricular mass index. The effective orifice area index was 1.00 ± 0.11 and 1.08 ± 0.14 cm²/m² in recipients of the 21-mm and 23-mm prostheses, respectively.

Size 21 mm and 23 mm Sorin Bicarbon prostheses show low transprosthetic gradients, with significant reduction of left ventricular mass index during the first postoperative year. The reported effective orifice areas might be useful for aortic valve replacement in patients with a small aortic annulus to avoid patient-prosthesis mismatch.

Aortic valve resistance has been proposed to represent the severity of aortic stenosis because some studies observed that it was less affected by change in flow than the valve-effective orifice area, but this issue remains controversial. The objective of this study was to systematically analyze the theoretical and practical determinants of these parameters in relation to changes in flow. Valve area and resistance in different valves were studied in vitro in a pulse duplicator system at different flow rates and in vivo in 90 subjects referred to either exercise or dobutamine infusion. Theoretical analysis and experimental results both demonstrated a unique relation between resistance (RES), valve-effective orifice area (EOA), and flow rate (Q): RES = K × (Q/EOA²). Accordingly, in fixed stenoses or in mechanical valves, resistance increased markedly with flow rate both in vitro (+0.88 ± 0.26%/% of flow increase) and in vivo (mechanical valves: +2.09 ± 4.61, fixed stenotic valves: +0.59 ± 0.32%/%), whereas valve area did not change significantly (<0.2%/%). In contrast, in valves with a flexible orifice (bioprostheses and some patients with aortic stenosis), resistance was less increased due to the increase in valve area. Thus, both from a theoretical and a practical standpoint, valve resistance is much more flow dependent than valve area, particularly in fixed stenoses. Situations in which resistance does not increase with flow rate are unpredictable and are found in flexible valves when there is a concomitant increase in valve area.

Left ventricular (LV) mass is a powerful predictor for future cardiovascular events. Epidemiologic studies have shown that hyperlipidemia is associated with higher LV mass. The effects of statin therapy for hyperlipidemia on LV mass have not been studied. To determine the effects of statin therapy on LV mass, we prospectively studied 3 groups of age and body surface area-matched patients: group 1 (n = 20), patients with systemic hypertension and hyperlipidemia treated with pravastatin plus antihypertensive drugs; group 2 (n = 20), patients with hypertension and hyperlipidemia treated with hypertensive agents and diet control alone; and group 3 (n = 20), hypertensive patients with normolipidemia treated with antihypertensive agents. A group of controls without hypertension or hyperlipidemia was used for comparison. Echocardiograms were recorded at baseline and after 6-month therapy. All hypertensive groups showed significant decreases in LV mass index after treatment. Group 1 had the greatest decrease in LV mass and it was significantly higher than in groups 2 and 3. Multivariate analysis revealed that regression of LV mass was significantly correlated only with the use of statins and sex (p = 0.005 and 0.01, respectively, R² = 0.47). Linear regression analysis in group 1 showed a significant correlation between changes in arterial compliance and LV mass regression (r = 0.57, p = 0.01). Thus, the addition of a statin may have an additional effect on reducing LV mass, independent of lipid-lowering effects.
