Infective endocarditis in intravenous drug abusers and HIV-1 infected patients

Influence of HIV-1 infection in endocarditis in IVDAs

Currently, the prevalence of HIV-1 infection among IVDAs with IE ranges between 40% and 90% [40], [73], [74], [80], [98], [99], [100], [101]. The full consequences of HIV infection in IE in IVDAs are not yet fully known because only a few studies have been published on this topic [98], [99], [100], [101], [102], [103]. The main conclusions from the literature review are the following.

Infective endocarditis in HIV-infected patients not related to IV drug abuse

IE in HIV infection occurs almost exclusively in IVDAs; it is very rare in other HIV-infected patients despite the fact that the number of HIV-infected patients is increasing worldwide [108], [109], [110], [111]. Some case reports have been published [25], [112], [113], [114], [115], [116], [117], [118], [119], [120], [121], [122], [123]. Table 7 summarizes the main characteristics of 22 cases of endocarditis in HIV-1-infected patients not related to IV drug abuse [108]. Eight cases were

      The complexity of these patients is evidenced by the prolonged length of hospital stay and the fact that many required transfer to another hospital or rehabilitation facility to facilitate recovery. Consistent with other series, we note that there is a higher prevalence of S. aureus and polymicrobial infections.10,21-24 The latter explains the need for multiple antibiotic agents in a significant proportion of cases.

      HIV infection among IVDA had dramatically increased (1600%) during the time of economic crisis as a result of the decline in gross domestic product, immigration waves, and network transmission (Paraskevis et al., 2013). According to data from other countries, the outcome of patients with IVDA IE is independent of the presence of HIV infection (Miró et al., 2002; Fernández Guerrero et al., 2009). In the current cohort, IVDA IE rates (10%) corroborated the international tendency (Gray et al., 2018) and was increasing compared with previous country studies (Giannitsioti et al., 2007; Loupa et al., 2004).

Drug abuse is a well-known risk factor for infection including bacteremia, endocarditis, and septic arthritis [38]. However, it has not previously been studied in open fractures.
