    Clinical introduction

    A young woman presented with paroxysmal palpitations. These episodes were sudden onset, sudden offset and not related to physical exertion, lasting 30 min to an hour. Four years ago, she received an ECG patch monitor for 3 months, but no events were captured. Physical examination was unremarkable. Resting 12-lead ECG showed sinus rhythm with no pre-excitation. Echocardiography showed structurally normal heart. Six months later, the patient had another recurrence of palpitations and this was captured on the Apple Watch Series 5 (figure 1).
