A 56-year-old man was referred for an electrophysiology opinion following an unexplained episode of syncope leading to a motor vehicle accident. Routine baseline investigations including an echocardiogram were normal. Following admission, the telemetry strip below was recorded (Figure 1). What is the mechanism of tachycardia?
