    An 85-year-old man presented acutely with NYHA class IV symptoms requiring continuous intravenous infusion of diuretics over several weeks. Echocardiography showed preserved left ventricular function and a severe central jet of regurgitation through a stented bioprosthetic aortic valve (25 mm Carpentier Edwards, implanted in 1994, Edwards Lifesciences Ltd, Irvine, California, USA) (figure 1A,B, videos 1 and 2).
