A 48-year-old lady presented with dyspnoea and chest pain. ECG showed sinus tachycardia with right bundle branch block and right ventricular strain. Echocardiography showed a dilated left ventricle with normal wall thickness and incoordinate septal motion with a globally poor systolic function (ejection fraction of 20%).
