Clinical introduction 

A 62-year-old woman presented herself to the Cardiology Department with a 3-month history of repeated episodes of sudden-onset palpitations and dizziness. Physical examination revealed a 2/6 holosystolic murmur at the apex. ECGs showed frequent premature ventricular contractions (PVCs). On Holter ECG monitoring, PVCs accounted for 21.9% of all heart beats and an episode of non-sustained ventricular tachycardia was recorded. Transthoracic echocardiography showed a dilated left atrium, moderate mitral regurgitation and preserved left ventricular (LV) function.
