There was no significant medical history and he took no regular medication. Twenty-four hours Holter monitoring revealed sinus rhythm with frequent ventricular ectopy. Echocardiography identified a wall motion abnormality at the apex, raising the possibility of ischaemic heart disease. Coronary angiography was subsequently performed, and this demonstrated normal coronary arteries.
