The new NICE guidelines for the assessment and diagnosis of patients with chest pain of recent onset place emphasis on coronary computed tomography (CT) for the evaluation of patients with an estimated likelihood of coronary disease between 10% and 29% and functional testing with myocardial perfusion stress imaging with SPECT, stress echocardiography and magnetic resonance (MR) perfusion imaging for patients with a likelihood of coronary disease from 30–60%. This is a significant change in the paradigm of current practice in which exercise stress testing features heavily and is accessible to all.
