Categorisation of economic level of country was based on information from World Bank in 2010, which represented the mid-interval of recruitment to the study. As the largest proportion of countries belonged to the upper middle-income category, we divided this category into two groups separated at the mid-point of GNI, with four categories of HIC, UMIC-1, UMIC-2 and (LMIC/LICs), using GNI.
