In units without cardiac surgical cover, an agreed written protocol must be in place in order to ensure that timely surgical relief of a tamponade is possible. The time taken for a patient to sternotomy should be of a similar order to that possible with on-site surgical facilities where a surgical team is not on stand-by.	IIa	C
