Abstract

This case report describes an anomalous origin of the left coronary artery. Despite the origin of the vessel from the aorta, this anomaly results in sudden death from coronary insufficiency because of the unusual course of the vessel between the aorta and pulmonary artery. This coronary artery anomaly has been described four times before in case reports, but it is not generally recognized as an entity with fatal potential.

The ALMCA prevalence and management have been assessed in non-randomized (case reports, case series and review articles) studies only. A total of 264 cases were identified [3–77] and the results are shown (Fig. 1, Tables 1–4). Table 1 shows the demographics, clinical characteristics, diagnostic tools and ostial anatomy of the systematic review patients.

In both anatomic variants, the left main coronary artery has several possible routes to the left side: (1) anterior to the pulmonary artery, (2) posterior to the aorta, (3) within the intraventricular septum beneath the right ventricular infundibulum, and (4) between the great vessels. The first three are generally benign and have not been associated with exercise-related death, whereas passage of the left main coronary artery between the great vessels has been associated with exercise related death [15–18]. There are two possible mechanisms for the association between a right sinus left main coronary artery and sudden death in athletes [19,20].

Anomalous aortic origin of a coronary artery is a rare and potentially lethal congenital anomaly. Increased risk of exertional sudden death is well described with both ALMCA and ARCA [2, 3, 12]. The combined incidences of these defects are reported to be between 0.1% and 0.3%.
