The exact anaesthetic requirement deemed necessary to facilitate cardioversion has been debated ever since the original description of using thiopentone in the early 1960s.17 No or minimal sedation can be associated with serious psychological sequela,1 whereas increasing anaesthetic depth increases the risk of significant cardiorespiratory depression. Awareness and recall were recorded in half the studies reviewed by the Cochrane collaboration.

The cardiology literature offers examples of MAC performed by nonanesthesiologists. Elective cardioversion and electrophysiology procedures have been shown to proceed more smoothly with less patient discomfort or prolonged sedation when balanced monitored anesthesia care is provided.21,32 A recent article from the Cleveland Clinic summarizes their experience with 1473 consecutive elective cardioversion procedures in 1041 patients.66
