Percutaneous Transvenous Mitral Commissurotomy (PTMC), also known as percutaneous mitral balloon valvuloplasty, was developed in 1984 for the treatment of selected patients with MS.16,17 It is a minimally invasive procedure performed under local anesthesia. Balloon dilation of the stenotic mitral valve improves hemodynamics by decreasing the transmitral gradient and thereby, reducing pulmonary artery and pulmonary capillary wedge pressure.18 It is associated with significantly less maternal and fetal complications and mortality compared to open mitral valve commissurotomy.19

The magnitude of pulmonary vascular pressure (PVP) is related to the degree of mitral stenosis, responsive contraction of pulmonary small arteries, and structural remodelling of the pulmonary vascular tissues [1]. One of the main reasons for persistent pulmonary hypertension is the abnormality of the structure of pulmonary vascular tissues [2]. Previous studies mostly focused on the pathologic changes of the small pulmonary vessel.
